Job Description

Cheyenne Village, Inc., a local 501(c)(3), the largest and oldest provider of residential services for adults with intellectual and developmental disabilities in Colorado Springs is seeking qualified applicants for a Service Coordinator.  This position offers you the opportunity help an individual with IDD  by empowering them to have a voice in planning their life and supports.  The Service Coordinator will create the Individual Service and Support Plan (ISSP) based on the information received for the individual.

As a Service Coordinator you organize and develop service and support plans for individuals with intellectual and developmental disabilities (IDD) that promote full participation in community life and meets the needs of each person.  Insures that all programs meet the regulatory guidelines and time lines. Monitors and revises individual plans in an ongoing and flexible manner to ensure supports and activities result in meaningful outcomes and continual success.  Maintains records in compliance with the Healthcare Policy and Finance and Colorado Department of Health rules and regulations.

Identifies and utilizes community resources (creative choices) to meet each person’s needs.  

Provides training to providers in regards to ISSP on a regular basis. 

Meet with each individual on a monthly basis to obtain feedback and discuss the ISSP.

Coordinates and completes annual assessment and evaluation of each person’s needs, strengths, interests and challenges utilizing input from the expanded service team.  Establishes priority areas of need for each person based on input from the individual and their family/support people. 

Skills / Requirements

Must be detailed oriented and deadline focused.  Requires excellent listening skills.  Able to deal with frequent change; manage competing demands and focuses on solving conflict.  Able to manage difficult or emotional situations with the individuals receiving services.

Bachelor’s degree in Human Services field preferred or equivalent in experience. One year’s experience in counseling, program development or behavior modification. One year’s experience with persons who have a developmental disability or mental health diagnosis. Working knowledge of Microsoft Office applications and e-mail.  Maintains criminal and driving record acceptable with State of Colorado, insurance carrier and Cheyenne Village, Inc. requirements.  Equivalent combination of education and experience may be considered.
